Back to School: 3 forms can now be completed in PowerSchool

Returning families are encouraged to log into PowerSchool as soon as possible to complete three required back-to-school forms:

  1. Student Information (including Emergency Contacts)
  2. Health & Medical Information
  3. Permissions and Agreements

Please note: These forms are for returning students entering grades 1-12 only. New students, including new kindergarten students, already completed these forms during registration.

Student schedule information will remain locked in PowerSchool until all three forms are submitted. Schedules will be released as follows:

  • HIGH SCHOOL: Course enrollments are available now. Complete schedules, with period, teacher and classroom assignments, will be released on August 7
  • MIDDLE SCHOOL: Schedules will be released on July 31
  • ELEMENTARY SCHOOL: Student classroom and teacher assignments will be released on August 12

It is vital that these forms are completed annually to ensure we have complete, up-to-date and accurate information for each student.

Emails on file for each student are used to communicate emergency alerts and important updates. They are also added to our distribution lists for district and school newsletters. Additionally, we are rolling out School Dismissal Manager (SDM) for all students this school year and a valid, working email address is required or you will not be able to access this system. Please review the first form, Student Information, carefully, ensuring all fields are complete and accurate.

TO COMPLETE THE FORMS

Log in to your PowerSchool parent account. If you do not have a PowerSchool parent account, you must create one as detailed here. Please note: If you use your student’s account to access PowerSchool, you will NOT be able to see or complete the forms. You MUST have a parent account.

Web Browser

  • Log in to PowerSchool using your parent account at nhsd.powerschool.com
  • Click “Forms” from the left-hand menu options
  • Select the “General Forms” tab (if not preselected) and you will see the above three forms
  • Complete and submit each one in numerical order

PowerSchool App

  • Log in to the PowerSchool app (App Store or Google Play)
  • Click “More” from the bottom menu options
  • Select “Forms”
  • Change the dropdown to “General Forms” and you will see the above three forms
  • Complete and submit each one in numerical order

Please note the following as you are completing these forms:

  • Once each form is submitted, changes cannot be made. Each form may only be submitted once.
  • Once submitted, you may see a warning message that the form is pending approval. This is what you should see and means that you have submitted the form correctly.
  • Only the first form needs to be approved by the school this year. Please do not be concerned if the form is not approved immediately.
  • The forms must be submitted before classroom and teacher assignments are unlocked and viewable when posted.

Please complete these forms ASAP to avoid delays in seeing your child’s schedule when available. The forms have been submitted successfully when you see the following:
